Microsoft Dynamics CRM 2011: Error after applying Update Rollup 12/13

I haven’t updated my VM to latest updates for a long time and I decided to do that today.
After Rollup was installed I saw message that Rollup was not applied to one or several organizations.
 I ignored that message and tried to log in to CRM. I have got following error:

Rollup issue: A duplicate value cannot be inserted into a unique index.

Hello!

On the week I faced with problem when MS CRM client can`t connect into CRM SERVER. This issue you can found on the 11, 12 and 13 Rollups.

Error log message looked like that:

Error| Exception : A duplicate value cannot be inserted into a unique index. [ Table name = MetadataSchema.EntityClientStatistics,Constraint name = PK__MetadataSchema.EntityClientStatistics__000000000000071C ]    at System.Data.SqlServerCe.SqlCeCommand.ProcessResults(Int32 hr)
   at System.Data.SqlServerCe.SqlCeCommand.ExecuteCommandText(IntPtr& pCursor, Boolean& isBaseTableCursor)

Big table AsyncOperationBase, resolution.

Hi every body!

On the week I faced with problem when my MS CRM 2011 has been working very slow. And I investigate what can be reason and found that, the AsyncOperationBase table has 25 Gb in SQL !

How can resolve this issue you can see in knowledge base the KB968520 by Microsoft. I hope this help you.

I'd like to give an update on the Microsoft Dynamics CRM 2011 Update Rollup (UR) 12 Release. We originally made UR12 available on Thursday, January 10th, to deliver multi-browser support for our On Premises customers. After discovering an issue that could potentially impact a customer's database, we withdrew the UR12 Server bits to ensure that no On Premises customers would be affected. Unfortunately, these bits were available on the Microsoft Download Center for a short period of time. If you downloaded the UR12 Server bits, please do not install them. We plan to repost the UR12 Server bits within the next week, and we will keep you informed as to when they are available. We have taken measures to improve our engineering processes and methodologies going forward, and we take your feedback very seriously. We apologize for any inconvenience this has caused.
